An Advanced Care Plan is a summary of a capable adult’s wishes detailing the kind of care they want or do not want in the event that they cannot speak for themselves. It sets out a person’s preferences about health and personal care and preferred care settings.

We can’t forecast what lies ahead but we all can take some steps to manage our future health care decisions. One of the ways is to prepare an Advance Care Plan and share it with the people involved in your life.
